Diabetes and Pregnancy

Diabetes Education and Dietitian referrals for women with Gestational Diabetes and women with pre-existing type1 or type 2 Diabetes who are pregnant.

General Practitioners

General Practitioners are requested to refer via First Call (Intake)

– Referral letter to First Call fax 56230856.

– Identify clearly, reason for referral (GDM / Type 1 diabetes in pregnancy / Type 2 diabetes in pregnancy)

– Identify clearly, that the patient is aware of diagnosis prior to referral being sent.

– Identify clearly, Patient Name, DOB, Address and Contact Number.
